Best way to use a touch up pen for a tiny rust spot?

So I open my door to notice a TINY spot of rust and paint chip on my driver door. I was able to snag a free paint pen for car at purchase from the dealer but I've never use it to cover a rust spot. If I don't remove all the rust would I be shooting myself in the foot? It is so small I rather not sand paper it down but I also don't want to let it spread and ruin things. Should I sand the area and use the paint pen after exposing the area?
Other suggestions?

I will have to inspect the pen. Tomorrow in my area will be warmer and dryer so maybe that will be the day. Would iron remover help remove some of the rust at least or should I go straight for the sand/file?

Edit: I did find a small handheld Dremel kit, I won't use the Dremel itself but it does have small stiff brush and a sanding but that I can use by hand to remove rust and expose the metal.
